Anal Fissure

Physical therapy may not be the first thing you think of for an anal fissure, but by treating the pelvic tension and bowel mechanics behind it, we focus on lasting recovery and preventing it from returning.

What is Anal Fissure?

An anal fissure is a small tear or crack in the lining of the anus, often caused by passing hard or large stools. This sensitive tissue has many nerve endings, which is why fissures can be extremely painful. They can make daily activities — such as sitting, walking, or using the bathroom — uncomfortable and sometimes even fearful. While they can occur at any age, they’re most common in adults and can develop suddenly or become a recurring issue if the tissue doesn’t heal fully. Having both a colorectal surgeon and a pelvic health physical therapist on your team are imperative to addressing your anal fissure and the underlying causes.

Common Symptoms

How Does It Feel?

  • Sharp, burning pain during or after a bowel movement
  • Bright red blood on toilet paper or in the toilet bowl
  • Muscle spasms around the anus
  • Itching or irritation in the anal area
  • Persistent discomfort when sitting for long periods

HOPE

What Causes It?

Anal fissures often occur when the anal lining is stretched beyond its limit. Common causes and contributing factors include:

  • Passing hard or large stools from constipation
  • Chronic diarrhea
  • Straining during bowel movements
  • Childbirth-related injury to the anal area
  • Tight or overactive pelvic floor muscles increasing pressure on the anal canal
  • Reduced blood flow to the area, which slows healing
  • Inflammatory bowel conditions or infections

How We Identify the Root Cause

Our evaluation begins with a thorough conversation about your symptoms, medical history, and lifestyle factors. We use a gentle approach to assess pelvic floor muscle function, coordination, and tissue health. If needed, we may collaborate with your gastroenterologist, colorectal surgeon, or primary care provider to rule out other conditions and ensure a complete understanding of the cause. By identifying not only the fissure itself but also the underlying contributors — such as muscle tension, bowel habits, or diet — we can develop the most effective plan for long-term healing.
